Here's a hand that I played atrociously.

LHO lead a low heart. Naturally, I played him for the jack, and consequently caught the king. Now I played the king of spades, crossed to the dummy with a heart, and pitched a heart with the ace of spades. What a mistake! I was always able to ruff the heart, so there was no need for the discard. I knew this, but what I didn't realize is that I would be able to develop spades so that I'd have 2 pitches for my clubs.

This may have been avoided if I had envisioned a possible layout of the hand, thus realizing that spades were likely 4-3.
